Driver Change Request Form

Please note: Drivers must be submitted for approval prior to being added to the policy. Please see the bottom of this form for more information on the driver approval process.

When requesting to add/pre-approve a driver:
**Please provide a copy of the MVR and include any documentation such as an old CDL picture or old MVR with a CDL issue date of 2 or more years ago. You may also provide a PSP report that shows a violation 2 or more years ago.**

**We will need these documents in order to properly review the driver. If for some reason you are unable to obtain this information or if you have any questions, please let us know.**
